Hemostatic net and hematoma prevention in facelift

Hematoma, a collection of blood under the lifted skin, is the most common early complication of a facelift. Surgeons lower the risk by controlling blood pressure, by drugs such as tranexamic acid, and by the hemostatic net, a row of temporary stitches through the skin that closes the space where blood can collect.

Hematoma rates

  • Hematoma is the most common major complication after facelift; 1.1% of 11,300 CosmetAssure facelifts (2008-2013) had a hematoma needing ER visit, admission or reoperation (PMID 26578747).
  • Male sex carried a relative risk of 3.9 for hematoma in that database (PMID 26578747).
  • 1,078 consecutive facelifts (1994-1999): hematoma 4.2%; independent risk factors were anterior platysmaplasty, systolic pressure, male sex, aspirin/NSAID use and smoking (PMID 11513508).
  • Men, 985 cases over 30 years: hematoma needing evacuation fell from 8.7% to 3.97% after a strict blood pressure regimen; women 1% to 3% (PMID 16327611).
  • Deep-plane facelift meta-analysis, 31 studies, 8,841 patients: overall hematoma 2.7%, major hematoma 0.97% (PMID 41203255).

Blood pressure control

  • Postoperative hypertension was shown to be a key cause of hematoma in 1976 (PMID 16327611).
  • 1,089 facelifts with a clonidine-based protocol and a systolic target of 140 mmHg or less had a hematoma rate of 0.9% (PMID 25811571).
  • 80% of the patients who bled in that series had systolic pressure above 140 mmHg in recovery (PMID 25811571).
  • The same 1,089-patient series describes the anesthesia protocol: anxiolysis, antiemetics, antihypertensives and smooth emergence (PMID 25415274).
  • Prophylactic postoperative medication (analgesic, antihypertensive, antiemetic, sedative) given before extubation: hematoma 0% (68 patients) against 7% when given on request (308 patients) (PMID 20333520).
  • Continuous monitoring, 229 patients: preoperative systolic pressure above 160 mmHg and male sex were independent risk factors (PMID 24912427).
  • 118 consecutive facelifts: labile intraoperative systolic pressure (swing of 80 mmHg or more) and high immediate postoperative systolic pressure predicted hematoma; the senior author targets 90-100 mmHg intraoperatively (PMID 39163270).
  • Clonidine review, 2026: supporting evidence is mostly Level IV (PMID 42434376).
  • Evidence review, 45 articles: prophylaxis of pain, nausea and hypertension, fibrin sealant and systolic pressure below 140 mmHg reduced hematoma; drains, compression dressings and steroids did not (PMID 37540899).

Tranexamic acid (TXA)

TXA is an antifibrinolytic drug given intravenously or mixed into the local anesthetic (PMID 40040948).

  • IV TXA randomized pilot, 44 deep-plane facelifts: lower surgeon-rated bruising and fewer postoperative collections; intraoperative bleeding difference not significant (PMID 32173730).
  • Split-face study, 70 patients: less bruising at 1 week on the side infiltrated with TXA (PMID 39574336).
  • IV (1 g) against local (150 mg in tumescent) randomized, 60 patients: equal blood loss; drains out sooner with IV (1.16 vs 2.04 days) (PMID 40040948).
  • Meta-analysis 2024: TXA lowered minor hematoma (OR 0.18) and drain output; no significant change in major hematoma (PMID 39141514).
  • Meta-analysis 2026, 7 studies, 1,122 patients: local TXA lowered blood loss, drain output and hematoma (OR 0.35) (PMID 42689241).
  • Wound-healing signal: 4 cases of wound complications attributed to local TXA were reported in 2023 (PMID 37265094).
  • Matched cohort, 261 vs 261 patients: local TXA at 1-2 mg/mL showed no increase in wound-healing complications (PMID 40611665).

The hemostatic net

  • Described by Auersvald and Auersvald: a running transfixing 5-0 nylon suture through skin and SMAS-platysma over the undermined area; hematoma 0 of 405 with the net against 17 of 120 (14.2%) controls; ischemia and necrosis rates unchanged (PMID 23949130).
  • Independent replication, one UK surgeon: hematoma 0.6% with net (304 patients) against 3.9% historical controls (359) (PMID 37040449).
  • Perfusion, clinical: laser-assisted fluorescence angiography (SPY-Q) before and after net placement in 8 cervicofacial flaps showed no significant change in perfusion (PMID 36636947).
  • Perfusion, cadaver: angiography and dye studies in 14 hemifaces showed no interference of the net with the subdermal plexus (PMID 37675581).
  • Perfusion, rat flap: net did not change flap survival; neovascularization at the flap base was higher (PMID 39834174).
  • Randomized trial, 160 deep-plane facelifts: net and drain had no significant difference in hematoma, seroma or edema (PMID 40131398).
  • Darker skin: 300 Thai patients, no persistent hyperpigmentation; 90% of suture marks gone within 1 month (PMID 39623234).
  • Internal net variant: internal quilting sutures in 41 patients with Fitzpatrick III-V skin, no hematoma (PMID 41181304); inner hemostatic net, 480-case report (PMID 40498628).
  • Carotid depth: 20.5-32.9 mm from skin at C2-C4 on CT; a 24 mm 3/8-circle needle gives more margin than a 32 mm needle (PMID 41992563).
  • Meta-analysis, 8 studies, 1,617 patients: hematoma 0.14 per 100; hyperemic suture marks 12.7 per 100, resolved in 2-3 months (PMID 41936137).
  • Meta-analysis, 11 studies, 3,393 patients: pooled hematoma 0.6% with net; comparative risk ratio 0.15 not significant; certainty low to very low (PMID 42154899).
  • Meta-analysis, 7 studies, 1,919 patients: odds ratio for hematoma 0.085 with the net (Caimi et al, Aesthet Surg J Open Forum 2025, https://academic.oup.com/asjopenforum/article/doi/10.1093/asjof/ojaf082/8176837; not found in PubMed on 2026-09-25).
  • Deep-plane meta-analysis: major hematoma 1.53% with TXA, 1.25% with sealants, 1.23% with nets; no adjunct clearly superior (PMID 41203255).

From hemostatic net to "surgical net"

  • O'Daniel and Kachare (2025) call it the "surgical net" and describe uses beyond hematoma: progressive tension, skin redraping and "gliding surgery" (PMID 38806160).
  • Gliding brow lift: subcutaneous forehead release through small incisions, brow held by the net; 118 of 124 patients with adequate repositioning (PMID 31511923).
  • Gliding brow lift, independent series: 63 patients, lift held at 12 months, 3.09 mm at the lateral canthus (PMID 39487802).
  • Extended gliding brow lift, 122 patients: tail of brow raised 3.2 mm (PMID 41217905).
  • Glidelift (periorbital and midface, no visible scar), 53 patients (PMID 41062668).
  • Chin ptosis correction with a 5-0 polypropylene surgical net, 20 patients, FACE-Q chin score 34 to 86 (PMID 40064646).
  • Breast surgery use: 24 patients, net used for skin redraping and fold stabilization (PMID 35294963).

"The surgical mesh": what it refers to

Steve's term "the surgical mesh" most likely means the hemostatic net, which O'Daniel's group now names the "surgical net" (PMID 38806160).

  • The published term is "surgical net" (PMID 38806160); the net is a running transfixing suture pattern across the undermined skin (PMID 23949130).
  • PubMed search on 2026-09-25, (hemostatic net OR surgical net OR Auersvald net) in title/abstract, 2021-2026: 39 records, including 3 meta-analyses (PMID 41936137, PMID 42154899, PMID 41203255) and 1 randomized trial (PMID 40131398).
  • The same search for mesh, acellular dermal matrix, AlloDerm or GalaFLEX combined with facelift, neck lift or platysmaplasty, 2021-2026: 4 records, none a clinical mesh series (search string in Open questions).
  • Implanted mesh in the neck is reported in one 10-patient series (Vicryl, Vypro, TIGR, AlloDerm slings); one Vypro mesh caused stiffness and was replaced (PMID 29794700).
  • Absorbable P4HB mesh reinforced SMAS in a cadaver tensile study; no clinical outcome data (PMID 24676588).

Other adjuncts

  • Drains: randomized split-face trial, 50 patients, no effect on hematoma; less bruising on the drained side (PMID 17572574).
  • Removing adrenaline from infiltration lowered hematoma needing evacuation in 461 facelifts (PMID 14707663).
  • Tissue sealants: meta-analysis of 7 RCTs, hematoma RR 0.29 (PMID 36260353).
  • Quilting sutures (internal or external): meta-analysis of 2 comparative studies, hematoma RR 0.02 (PMID 31784752).
